A medical provider must clearly support
your requirement for the accommodation request.
Once documentation has been received and reviewed, an email update will be sent to the student’s email address.
If approved, the student will be notified of the next steps.
Submitting an
ADA Accommodation Request
Log in to the USH Resident Portal
- Select Apply for Housing from the menu bar
- Select ADA Accommodation Request
- Complete the questions and submit
- Download the ADA Accommodation Form
- Have your medical provider fill out the form
- Email the completed form to admin@ushcommunities.org

A request may be submitted at any time once a student has an application. The earlier a request is made, the more options there may be to accommodate the student’s needs.
Accommodation approvals may impact other housing preferences, such as the location of an apartment or room. Priority will be given to the apartment or room’s suitability in the accommodation process over additional preferences.
If an accommodation is approved but no room is currently available, they will be placed on a preference list until a suitable space opens.
Approval for an ADA Accommodation is for the academic year you receive approval for only.
Accommodation requests must be submitted every year.

University Student Housing is committed to supporting individuals with disabilities through reasonable accommodation. The below process is for USH affiliated housing only.
USH approved ESA’s are not permitted in other areas of the University without prior approval from the Office of Educational Accessibility (OEA).
Submitting an
Assistance Animal Accommodation Request
Log in to the USH Resident Portal
- Select Apply for Housing from the menu bar
- Select ADA Accommodation Request
- Click the Academic Year
- Choose Emotional Support Animal or Service Animal
- Fill in animal information, veterinarian information, and emergency contact information
Once ADA documentation is received and reviewed, you will receive email with approval/denial and next steps. If approved, the student is required to:
- Send up to date vet records to: admin@ushcommunities.org
- Sign an On Campus Animal Agreement via their resident portal with their guarantor in order to bring animal to campus
- Choose Emotional Support Animal OR Service Animal
- Fill in Animal Information, Veterinarian Information, and Emergency Contact Information
Assistance Animal Accommodations
To ensure that USH meets the specific needs of students who require an ESA or service animal while living on campus, students should follow the Assistance Animal Process for approval.
